Ceresti Health, headquartered in Carlsbad, California, is a nationwide CMS GUIDE Model participant serving families across all 50 states. Eligible Traditional Medicare beneficiaries and caregivers receive a dedicated care navigator, personalized dementia education, caregiving tools, and up to $2,500 per year in respite care at no out-of-pocket cost. Services are delivered through an iPad-based platform with pre-loaded content, internet access, and engagement modules including reminiscence and music therapy.
EmpowerMe Medical Group, P.C., the medical practice of EmpowerMe Wellness, participates in the Medicare GUIDE Model, serving beneficiaries in all 50 states. The University of Connecticut Health Center's James E. C. Walker Memory Assessment Program diagnoses Alzheimer's and other dementias in Farmington, Connecticut.
